Online access

There are several options for accessing GP services digitally. Some of the most popular apps are listed below, but others are available. First you must complete the online access request form which confirms the level of access you wish to have. Please print, sign in ink, and deliver the hard copy to the practice. If you do not have access to a printer, you can collect a blank form from the practice.

Using NHS Login or obtaining a linkage key

The NHS app and Evergreen Life can both use the NHS login which you can set up from home without coming to the surgery, and can give you full access to your record as your identity is verified from official Government records. Other apps including Patient Access requires a linkage key which can only be obtained after we have verified your identity. We can now do this by telephone or video call instead of requiring attendance in person with photo ID. Once you have sent in the access request form we can then arrange for the telephone or video verification call.

Proxy Access

If access is required for someone else's medical record - for example: if you are a carer; for an elderly relative who doesn't have internet access; or for your child - Proxy Access allows someone to act on behalf of the patient. Level of access is tailored to suit the situation and can be limited or expanded depending on need. Appointment booking, repeat medication, and access to medical records, can be accessed by the proxy once the person has been linked to their account. To arrange Proxy Access download and the form linked below AND the online access form above, print and sign and drop in to surgery. We require a "wet" signature in hard copy for these documents.
